Your insurance carrier likely covers pediatric evaluations and therapy. Contact them to understand your plan and coverage. Please visit 'Physicians' to see a list of our current accepted insurance plans.
Some plans require a referral from your child's pediatrician for services. Please find our referral form under 'Physicians' labeled "Physician Order Form_TTC"
Contacting our center is the first step in getting your child's evaluation scheduled. We are available to assist you in obtaining a script from your child's pediatrician in order to schedule the evaluation. Below are the steps you will need to complete before your child's first appointment:
1. Obtain a script from your child's pediatrician. Thrive's script forms are conviently located under the 'Physicians' tab to download.
2. Your child's pediatrician will fax the script, or you are able to email to our HIPAA-complaint address, or bring in a hard copy to your first appointment.
3. Complete the paperless intake forms sent to you via email from SimplePractice's Client Portal.
Upon receiving a script from your child's pediatrician, our staff will verify any insurance coverage and contact you to schedule an evaluation and complete our paperless intake process.
